THE MMA FARM INC. – PARTICIPANT WAIVER, RELEASE OF LIABILITY & ASSUMPTION OF RISK
PLEASE READ CAREFULLY. BY SIGNING THIS DOCUMENT, YOU ARE WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE.
1. ACKNOWLEDGEMENT OF RISKS
I, the undersigned, acknowledge that I am voluntarily participating in physical training, martial arts instruction, combat sports, and conditioning activities provided by The MMA Farm Inc., including but not limited to:

Technical No-Gi Jiu-Jitsu, submission wrestling, grappling, and joint manipulation.

Kickboxing, Muay Thai, Boxing, and Mixed Martial Arts (MMA) striking.

Live contact sparring, drilling, strength & conditioning, and functional movement exercises.

Use of gym facilities, mat areas, heavy bags, training equipment, locker areas, and premises.
I understand and acknowledge that martial arts and combat sports are inherently physical and high-risk activities. Risks include, but are not limited to: minor injuries (cuts, bruises, sprains, strains); severe injuries (bone fractures, joint dislocations, torn ligaments, concussions, head/neck trauma); transmission of skin infections or communicable illnesses; soft tissue damage; permanent paralysis; or death.
I freely accept and fully assume all such risks, dangers, and hazards, and the possibility of personal injury, death, property damage, or loss resulting therefrom.
2. WAIVER AND RELEASE OF LIABILITY
In consideration of The MMA Farm Inc. permitting me to participate in its programs and utilize its facility, I HEREBY AGREE TO WAIVE ANY AND ALL CLAIMS, demands, actions, or causes of action that I have or may in the future have against The MMA Farm Inc., its directors, officers, employees, instructors, coaches, agents, representatives, independent contractors, facility owners/landlords, and successors (collectively referred to as the "Releasees").
I HEREBY RELEASE THE RELEASEES FROM ANY AND ALL LIABILITY for any loss, damage, injury, expense, or death that I may suffer, or that my next of kin may suffer, as a result of my participation in activities at or organized by The MMA Farm Inc., DUE TO ANY CAUSE WHATSOEVER, INCLUDING NEGLIGENCE, BREACH OF CONTRACT, OR BREACH OF ANY STATUTORY OR OTHER DUTY OF CARE (INCLUDING ANY DUTY OF CARE OWED UNDER THE OCCUPIERS LIABILITY ACT, R.S.B.C. 1996, c. 337) ON THE PART OF THE RELEASEES.
3. HEALTH, SAFETY & MEDICAL FITNESS

Physical Condition: I declare that I am in proper physical condition to participate in strenuous combat sports and physical training, and I have no undisclosed medical conditions, injuries, or illnesses that would endanger myself or others.

Safety & Hygiene Rules: I agree to strictly follow all instructor directions, gym safety protocols, hygiene rules, and equipment guidelines. The MMA Farm Inc. reserves the right to suspend or terminate membership for reckless behavior or failure to adhere to safety standards.

Emergency Medical Treatment: In the event of a medical emergency, I authorize staff of The MMA Farm Inc. to secure first aid or emergency medical transport on my behalf if I am unable to do so. I agree to assume full financial responsibility for any medical expenses incurred.
4. MEDIA RELEASE (OPTIONAL / STANDARD)
I grant The MMA Farm Inc. permission to use photographs, video footage, or digital media recorded during training sessions, open mats, or events for promotional, marketing, and educational purposes on social media, websites, and print materials, without compensation or royalties.
5. SEVERABILITY & GOVERNING LAW
This Agreement shall be governed by and interpreted strictly in accordance with the laws of the Province of British Columbia and the federal laws of Canada applicable therein. If any provision or portion of this agreement is found to be invalid or unenforceable by a court of competent jurisdiction, the remaining provisions shall remain in full force and effect.
ACKNOWLEDGEMENT OF UNDERSTANDING
BY SIGNING BELOW, I CONFIRM THAT I AM AT LEAST 19 YEARS OF AGE (THE AGE OF MAJORITY IN BRITISH COLUMBIA), THAT I HAVE FULLY READ AND UNDERSTOOD THIS AGREEMENT PRIOR TO SIGNING IT, AND THAT I AM AWARE THAT BY SIGNING THIS AGREEMENT I AM WAIVING CERTAIN LEGAL RIGHTS WHICH I OR MY HEIRS, NEXT OF KIN, EXECUTORS, AND ADMINISTRATORS MAY HAVE AGAINST THE RELEASEES.
